Grant Description

CommonSpirit Health, one of the largest nonprofit health systems in the United States, operates a comprehensive Community Health Improvement Grants program to address the critical health disparities and service gaps affecting vulnerable and underserved populations. With a national presence and deep local roots, CommonSpirit channels resources through this grants program to support collaborative efforts among nonprofit organizations that aim to improve health outcomes and advance equity within communities served by their hospitals. The grant program provides charitable, project-specific funding to 501(c)(3) nonprofit organizations, or to those with a 501(c)(3) fiscal sponsor, to undertake efforts aligned with their local hospital’s most recent community health needs assessment (CHNA) and implementation strategies. Proposals must address significant health needs and demonstrate how the funded activities will support health equity, increase access, or strengthen the local health service system. Projects must include continuous activity over a 9 to 12-month period and show a clear alignment with the needs identified in the CHNA. Notably, for proposals requesting $20,000 or more, applicants must include one or more collaborating partners, each with defined roles that contribute substantively to the project. Funding is disbursed as restricted charitable contributions and may not be used for unrestricted operating support, event sponsorships, short-term activities, capital projects, political campaigns, or substantial equipment purchases. Projects with capital costs, like building construction or renovations, are explicitly ineligible. All proposals must be for a one-year period, with the opportunity to reapply annually for the same or new projects. However, a project will not be funded for more than four consecutive years. Funding amounts vary by hospital but generally fall within two categories: small grants (up to $19,500) and standard grants ($20,000 to $150,000). Applications must be submitted online between July 14, 2025, at 9:00 AM PDT and September 12, 2025, at 11:55 PM PDT. The application portal is accessible via the "Hospital List for Applicants" link on the CommonSpirit Health website. Supporting materials available for applicants include the Hospital List, Community Health Needs by Hospital, FAQs, and Application Questions documents. An AmpliFund Applicant Portal Training video is also provided for technical assistance. Award notifications are expected by January 19, 2026, with grant agreements signed and payments disbursed by March 2026. The grant implementation period will begin April 1, 2026, and run through March 31, 2027. A mid-year project report is due November 13, 2026, and the final report must be submitted by May 16, 2027. While CommonSpirit hospitals in Utah, Colorado, and Kansas do not participate in this specific grant program, hospitals in other states under full CommonSpirit ownership are eligible to administer and distribute these grants.
