Services covered under this program include medical case management, medication and insurance premium assistance, medical and mental health services, oral and nutritional care, legal and linguistic services, psychosocial support, and emergency financial assistance. The grant also emphasizes virtual outreach to service professionals and the promotion of HIV Care Connect through digital media and organizational newsletters. Applicants will be responsible for measurable performance deliverables including virtual outreach to at least 500 providers, participation in at least four external virtual events, publication of at least eight articles, and achieving targeted digital engagement metrics. These metrics include attracting 2,500 unique visitors to the program’s website, gaining 500 social media followers, and securing placement of the HIV Care Connect website link on at least 12 partner websites. Eligible applicants are limited to tax-exempt and governmental organizations in good standing with the State of Illinois and federal authorities. Applicants must complete the Grant Accountability and Transparency Act (GATA) prequalification process, maintain an active SAM registration, and meet fiscal and legal standing requirements. Organizations with certain felony convictions or bribery violations may be disqualified unless sufficient time has passed or individuals involved are no longer part of the organization. All applications must be submitted through the Illinois Department of Public Health’s Electronic Grants Administration and Management System (EGrAMS) by March 5, 2026, at 4:00 PM CDT. The application process requires full completion of all components including a detailed budget in both the EGrAMS system and an attached Ryan White Part B Budget document. Mandatory technical assistance is scheduled for February 11, 2026, at 9:00 AM, and registration is required via the provided WebEx link. Post-submission, applications will undergo a merit-based review process based on criteria such as grant history, scope of work, and budget clarity. The grant performance period runs from April 1, 2026, to March 31, 2027. Indirect and administrative costs are capped at 10% of the award total. Successful applicants will be notified through EGrAMS and required to electronically sign a grant agreement. Grantees must utilize the state’s Provide Enterprise database for quarterly reporting, billing, and reimbursement, and ensure full documentation compliance.
