Illinois Asthma Program Large Asthma Consortium

The Illinois Department of Public Health (IDPH) is offering funding through the Illinois Asthma Program Large Asthma Consortium (IAPLAC) initiative, aimed at improving asthma outcomes across the state. This grant seeks to enhance the reach, quality, effectiveness, and sustainability of asthma control services while reducing disparities, morbidity, and mortality through guidelines-based asthma education, community engagement, and clinical-community linkages. The successful applicant will serve as a statewide hub for asthma education targeting stakeholders such as healthcare professionals, schools, community health workers, and community-based organizations. Activities will focus on promoting National Asthma Education Prevention Program (NAEPP) guidelines-based care, facilitating training and quality improvement initiatives, building clinical-community partnerships, and addressing health disparities. Key outcome goals include reduced hospitalizations, improved asthma control behaviors, reduced costs, and expanded access to asthma care in underserved populations. Applications must be submitted through IDPH’s EGrAMS system (https://idphgrants.com) by 5:00 PM Central Time on May 16, 2025. The anticipated award announcement is June 6, 2025, and the project period will run from July 1, 2025, through June 30, 2026. Up to $680,000 in total program funding is available, with only one award anticipated. Eligible applicants include Illinois-based entities capable of delivering statewide asthma education and promoting clinical-community linkages. Applicants must be prequalified through the Grant Accountability and Transparency Act (GATA) portal, have a valid SAM.gov registration, and not appear on any exclusion lists. No cost-sharing or matching is required. Indirect costs are allowed, provided there is a valid indirect cost rate or election of the de minimis rate. Grant activities will require participation in monthly meetings, reporting on quarterly and annual metrics, participation in the Illinois Asthma Partnership workgroups and meetings, and contributions to statewide asthma policy and practice improvements. Strong emphasis is placed on health equity, requiring applications to address the Health Equity Checklist and demonstrate culturally and linguistically appropriate services.
