Thriving Communities Grant Program

It also includes well-being initiatives targeting mental health, substance abuse treatment, and domestic abuse services. Additionally, grants fund efforts that preserve local culture and identity as well as projects enhancing community spaces through beautification and improvements that attract residents and businesses. Applicants are required to submit a valid Federal W9 form along with documentation proving tax-exempt status. For nonprofits, additional materials include an organizational overview highlighting achievements and impact, a board of directors list, recent IRS Form 990 filings, a budget for the proposed project or program, and the organization’s full-year operating budget.
