There will be only one award for this RFA. The planned RFA will utilize the UG3/UH3 activity code with details provided below. The planned RFA will support a Clinical Coordinating Center (CCC) and will run in parallel with a companion RFA for a collaborating Data Coordinating Center (DCC) application.This RFA will accept only an application that is part of a collaborative pair of applications. The pair must include one application to the CCC UG3/UH3 Notice of Funding Opportunity (NOFO) plus one application to the companion DCC U24 NOFO.Both a CCC application and a collaborating DCC application must be submitted on the same due date for consideration by NHLBI. CCC (UG3/UH3) applications submitted without a collaborative DCC (U24) application will be deemed incomplete and will not proceed to review. This Notice is being provided to allow potential applicants sufficient time to develop meaningful collaborations and responsive projects. The planned RFAs are expected to be published in Summer 2024 with an expected application due date in Fall 2024.

Black/African Americans have been disproportionately affected by HIV since the early days of the epidemic and that disparity has deepened over time. Although they represent only 12% of the population, Black/African Americans account for approximately 43% of new HIV diagnosis. This disparity is further exacerbated by the critical shortage of primary care providers trained to provide counseling, HIV testing, prescribe PrEP and HIV care and treatment. This is especially true for the number of providers working in communities most at-risk for HIV. The purpose of the Building Workforce Capacity through Pathways in HIV Residency Training Program (Pathways Program) is to train internal and family medicine residents in HIV treatment and care management with the goal of increasing the number and diversity of HIV providers.

Sodium-glucose co-transporter-2 inhibitors (SGLT2i) that were developed for the treatment of type 2 diabetes (T2D) have significant protective effects for cardiac and renal diseases for people with and without diabetes. However, SGLT2i are not currently approved for individuals with type 1 diabetes (T1D) and there is an increased risk of diabetic ketoacidosis (DKA) for this population. Despite this concern, these drugs are increasingly being prescribed off-label for people with T1D. Continuous ketone monitoring (CKM) is a rapidly evolving technology that could be utilized clinically to prevent DKA by an early warning of elevations in ketone levels.The purpose of this NOFO is to solicit applications for studies that will develop and test risk mitigation strategies that involve the clinical integration of CKM for the safe use of SGLT2i for people living with T1D so that they may benefit from the cardiac and renal protection and glucose-lowering effects of this drug class. The NOFO will support short-term, clinical trials to gain and disseminate knowledge on safety and glucose control with CKM and SGLT2i use.Possible topics include testing optimal insulin delivery in open and closed loop systems or multiple daily injections and developing clinical protocols to control or ameliorate elevated ketone levels in individuals with T1D who are receiving adjunct therapy with SGLT2i.
