Program Summary The PATH program is designed to reduce and/or eliminate homelessness for individuals who may be experiencing (SMI) serious mental illness and co-occurring substance use disorders. Our goal is to ensure that individuals are connected to mainstream MH services and eventually transitioned into stable housing improve their health, and live a self-directed, purposeful life. Funding Priorities or Focus Areas • IDHS is working to counteract systemic racism and inequity, and to prioritize and maximize diversity throughout its service provision process. This work involves addressing existing institutionalized inequities, aiming to create transformation, and operationalizing equity and racial justice. It also focuses on the creation of a culture of inclusivity for all regardless of race, gender, religion, sexual orientation, or ability. Project Description The Grantee will operate the Projects for Assistance in Transition from Homelessness (PATH) formula grant providing flexible, community-based services throughout the State of Illinois to address the needs of adults ages 18-65+ and families, with serious mental illness and co-occurring substance use disorders, who are homeless or at imminent risk of becoming homeless. PATH links the population of individuals with serious mental illnesses (SMI)/co-occurring substance use disorders (COD), who experience persistent and pervasive health disparities, to mainstream and other supportive services. Collectively the PATH resources help individuals experiencing or at risk for homelessness with SMI/COD secure safe and stable housing, improve their health, and live a self-directed, purposeful life.

This grant provides funding to counties to develop local programs and services that offer alternatives to incarceration for juvenile offenders, aiming to reduce commitments to the Illinois Department of Juvenile Justice by at least 25%.

Funding Information ​The Oregon Department of Energy has about $1.2 million available for grants to eligible local governments for energy efficiency, renewable energy, or transportation-related projects. The minimum grant for a qualifying community is $50,000. The maximum grant amount is $100,000. Period of Performance If awarded, a grant applicant’s Performance Agreement will outline the timeframe required to receive the grant award. In general, EECBG Performance Agreements are anticipated to provide 24 months from the date of the agreement for the project to be completed. This program provides funding to communities facing urgent health and safety threats due to recent disasters, helping them address needs that cannot be met through other funding sources.

The Pennsylvania HOME-American Rescue Plan Supportive Services (HOME-ARP SS) funds can provide a broad range of supportive services to individuals and families that meet one of the qualifying populations as defined in Notice CPD-21-10: Requirements for the Use of Funds in the HOME-American Rescue Plan Program. Supportive services may be provided to individuals and families who are not already receiving the services outlined in the Notice through another program. Applicants may establish a separate supportive services activity or activities or may combine supportive services with other HOME-ARP activities. Notice CPD-21-10: Requirements for the Use of Funds in the HOME-American Rescue Plan Program HOME-ARP funding is provided by the U.S. Department of Housing and Urban Development (HUD) and administered by DCED. The Tobacco Grant Program, administered by the Office of the Attorney General, California Department of Justice, aims to foster a healthier California by curtailing illegal cigarette and tobacco sales and marketing to minors. The California Energy Commission (CEC) is seeking applications to deploy advanced decarbonization and/or grid support technologies at California industrial facilities to promote electrification and reduce GHG emissions produced from the industrial sector. Applicants applying as a health care practitioner must include with their proposal a current, valid, unrestricted license to practice in the State of California. The Permanent Commission on the Status of Racial, Indigenous, and Tribal Populations (‘the Permanent Commission’ or ‘the Department’) is seeking applications to develop and implement a program or initiative that intentionally addresses at least one (1) SDOH and builds stronger community connections. The Rural and Underserved Clinical Rotations (RUCR) grant program, authorized by Minnesota Statutes Sec 144.1505, is established to award health professional training site grants to eligible physician, physician assistant, advanced practice registered nurse, pharmacy, dentistry, dental therapy, and mental health professional programs to augment existing clinical training programs to add rural and underserved rotations or clinical training experiences, such as credential or certificate rural tracks or other specialized training. Eligible Expenses Grant funds may be used to cover the cost of: establishing or expanding rotations and clinical training; recruitment, training, and retention of students and faculty; connecting students with appropriate clinical training sites, internships, practicums, or externship activities; travel and lodging for students; faculty, student, and preceptor salaries, incentives, or other financial support; development and implementation of cultural competency training; evaluations; training site improvements, fees, equipment, and supplies required to establish, maintain, or expand a training program; and supporting clinical education in which trainees are part of a primary care team model. Eligible Applicants  The following types of clinical training programs located in Minnesota are eligible to apply for this funding: Advanced Practice Registered Nursing Program Must be accredited as a master’s, doctoral, or postgraduate level advanced practice registered nursing program by the Commission on Collegiate Nursing Education or by the Accreditation Commission for Education in Nursing or present a credible plan as a candidate for accreditation. Dental Program Must be a dental education program or dental residency training program currently accredited by the accrediting body or present a credible plan as a candidate for accreditation. Expanded training must include rotations in primary care settings such as community clinics, hospitals, health maintenance organizations, or practices in rural communities. Dental Therapy Education Program or Advanced Dental Therapy Education Program Must be approved by the Minnesota Board of Dentistry, or currently accredited by the Commission on Dental Accreditation. Mental Health Professional Program Must be accredited and listed as a mental health professional program by the appropriate accrediting body for clinical social work, psychology, marriage and family therapy, or licensed professional clinical counseling, or present a credible plan as a candidate for accreditation. A mental health professional is defined as an individual providing clinical services in the treatment of mental illness who meets one of the qualifications under MN Statutes Sec. 245I.04, subdivision. Pharmacy Program Must be accredited as a Doctor of Pharmacy program by the Accreditation Council on Pharmacy Education. Physician Residency Training Program Must be accredited by the Accreditation Council for Graduate Medical Education (ACGME) or appropriate accrediting body or present a credible plan as a candidate for accreditation.
