The impact goals include improving public health outcomes, increasing social capital, and fostering stronger community connections within these communities. This program recognizes that persistent disparities in SDOH require targeted and community-driven solutions to achieve equitable health and well-being. The program prioritizes supporting CBOs in creating new and innovative community-led approaches to addressing SDOH for their communities. A key focus is on increasing social capital in historically marginalized communities by supporting community-building activities within Maine. Eligible applicants must be CBOs as defined by the RFA, meaning they are driven by and accountable to the community they serve, have a physical presence, and include community perspectives in determining priority issues and solutions. Proposed projects must specifically benefit communities within Maine. Expected outcomes include the establishment of stronger relationships between the Permanent Commission and CBOs/community leaders, direct investment of resources into severely impacted Maine communities, and the successful implementation of new, innovative, community-led initiatives addressing SDOH. Measurable results will likely involve tracking the number of CBOs partnered with, the reach of the programs, and indicators related to improved SDOH outcomes and increased social capital within the targeted communities. The total grant funding for this program is $300,000, with applicants able to apply for a minimum of $25,000 and a maximum of $75,000 over a contract period not exceeding 18 months or extending beyond December 2025. This funding supports the Commission's strategic priority of addressing health equity through community empowerment and direct resource allocation.

CMP In-Person Visitation grants provide funding for nursing homes to facilitate safer in-person visits through use of outdoor and indoor equipment such as tents, clear dividers, portable fans, and portable room air cleaners with high-efficiency particulate air (HEPA, H-13 or 14) filters in response to the COVID-19 pandemic. Replacement air filters are a prohibited expense under this program.  Facilities must attest they meet the requirements to purchase the materials and facilitate in-person visitation for residents. Facilities must be certified through the Centers of Medicare and Medicaid Services (CMS). Applicants may request up to a maximum of $3,000.00 per facility. In collaboration with CalVet, grantee will host the 2024 and 2025 CalVet Leadership Summit bringing together veteran and leaders across multiple relevant organizations to focus on improving the health and well-being of California's service members, veterans, and their families. The Leadership Summit plays a vital role in supporting the department’s strategic plan by providing a platform for leaders to come together and work towards common goals. ; CalVet seeks to organize stakeholder input workshop activities with the assistance of an organization that is highly experienced in serving transitioning veterans, developing and facilitating veteran stakeholder summits or workshops, and has the capacity and proven track record of advocating for veteran services and benefits. The selected Grantee will assume all responsibility for the Required Activities established in Section E for the 2024 and 2025, CalVet Leadership Summit(s) and will operate with prior understanding that unanticipated changes in CalVet priority may impact the size and scope of the relevant annual summit. Grantees will receive the full amount of funding for their awarded grant in the first year. It is expected that Grantee makes up the difference from its own resources in full actual costs, beyond which this Grant provides, with each passing year to sustain the Leadership Summit through the expiration of the Grant terms.The Grantee staff and leadership will be expected to maintain significant engagement with CalVet. Applicants are encouraged to ensure they are budgeting for staff, reporting, and appropriate facility requirements (venue, seating, lodging, hardware and software equipment, supplies, overhead).

The Texans Feeding Texans Home-Delivered Meal Grant Program aims to supplement and extend current home-delivered meal programs for seniors and/or disabled Texans. Eligible entities include governmental and non-profit agencies. The program involves counties making qualifying grants to meal providers based on a rate of $0.25 per county senior as reported in the 2020 census. Meal providers must receive a grant from the county, deliver meals to homebound elderly or disabled individuals, and complete a required application. The Texas Department of Agriculture facilitates the funding process.
