Eligible applicants must be Somerset County-based. The grant prioritizes organizations that provide programs or services that achieve these goals. Another key focus is on improving the operational and managerial effectiveness of local history organizations, thereby ensuring their long-term sustainability and impact. The foundation's theory of change appears to be that by empowering local organizations with financial support and clear objectives, they can more effectively serve the public and preserve historical assets. Expected outcomes include a more engaged public with New Jersey history, a greater depth of historical knowledge disseminated, and more efficiently run local history organizations. Measurable results would likely include increased participation in historical programs, documented improvements in organizational management, and successful preservation efforts of historic sites. Organizations are also required to have provided public history programs and/or services in Somerset County for the benefit of the general public for at least two years prior to application, and must provide proof of stability and fiscal responsibility, ensuring the grant's investment is directed towards established and accountable entities.

Utilizing CMP funds from the federal CMS, facilities will submit proposals for various quick improvement projects within their facility. In an effort to support the maintenance of cognitive and mobility skills, the Illinois Department of Public Health, Office of Healthcare Regulation (OHCR) announces an opportunity for certified facilities to submit applications for projects that must provide the residents of long term care facilities with a program or project deliverable that meaningfully improves the quality of life or quality of care for residents in the facility. These projects should deliver direct improvements to quality of life, direct improvements to quality of care, alternatives to pharmacological interventions, alternative behavioral management, training programs and implementation.

CMP In-Person Visitation grants provide funding for nursing homes to facilitate safer in-person visits through use of outdoor and indoor equipment such as tents, clear dividers, portable fans, and portable room air cleaners with high-efficiency particulate air (HEPA, H-13 or 14) filters in response to the COVID-19 pandemic. Replacement air filters are a prohibited expense under this program.  Facilities must attest they meet the requirements to purchase the materials and facilitate in-person visitation for residents. Facilities must be certified through the Centers of Medicare and Medicaid Services (CMS). Applicants may request up to a maximum of $3,000.00 per facility. The Texans Feeding Texans Home-Delivered Meal Grant Program aims to supplement and extend current home-delivered meal programs for seniors and/or disabled Texans. Eligible entities include governmental and non-profit agencies. The program involves counties making qualifying grants to meal providers based on a rate of $0.25 per county senior as reported in the 2020 census. Meal providers must receive a grant from the county, deliver meals to homebound elderly or disabled individuals, and complete a required application. The Texas Department of Agriculture facilitates the funding process.
