In awarding the contracts or grants, the Health Department shall prioritize Directly provided core public health services within the Hamilton County Health Department jurisdictional areas; Currently operational local health care providing entities including hospitals, clinics, physicians, pharmacies, and home health agencies; Evidence-based practices to achieving desired health outcomes, including the use of behavioral incentives; and Initiatives that contract collaboratively with proven outcome-based health improvement providers or services. Applicants are encouraged to propose projects, programs, or activities aligned with the following priority Core Public Health Service (CPHS) areas for Hamilton County: Maternal and Child Health Referral programs Improved Birth Outcome programs Chronic Disease Prevention Child & Adult Immunizations Access to and Linkage to Clinical Care School Health Trauma and Injury Prevention Fatality Prevention Initiatives Tobacco Prevention and Cessation Eligibility Criteria In accordance with IC 16-46-16-.7(2), eligible applicants include an individual, employer, employer association, nonprofit organization, for-profit organization, institution of higher education, health insurance plan, health ministry, or any combination thereof such as a health coalition providing core public health services in partnership with the Hamilton County Health Department.
