HRSA-25-002, HRSA-25-003, and HRSA-25-004 announces the opportunity to apply for funding under Ryan White HIV/AIDS Program (RWHAP) Part C Early Intervention Services (EIS) Program: Existing Geographic Service Areas. The purpose of this program is to provide comprehensive primary health care and support services in an outpatient setting for low-income people with HIV. Under this announcement, successful applicants must provide: (1) counseling for individuals with respect to HIV; (2) targeted HIV testing; (3) periodic medical evaluations of individuals with HIV and clinical and diagnostic services for HIV care and treatment; (4) therapeutic measures for preventing and treating the deterioration of the immune system, and for preventing and treating conditions arising from HIV; and (5) referrals for people with HIV to appropriate providers of health care and support services. These services are to be provided directly or through referrals, contracts, or memoranda of understanding (MOUs). This competition is open to current RWHAP Part C EIS recipients and new organizations proposing to provide RWHAP Part C EIS funded services in the geographic service areas listed in Appendix C. Please note that there are 3 funding announcement numbers included in this document with 3 different periods of performance (See Appendix C). If you are applying for more than one service area, you must submit a separate application for each proposed service area. All allowable services must relate to HIV diagnosis, care, and support, and must adhere to established HIV clinical practice standards consistent with U.S. Department of Health and Human Services (HHS) Guidelines. Please refer to the HIV/AIDS Bureau (HAB) Policy Clarification Notice (PCN) 16-02 Ryan White HIV/AIDS Program Services for a list of RWHAP allowable core medical and support services and their descriptions. According to the RWHAP Part C statute: At least 50 percent of the amount received under the award must be expended on EIS costs (except counseling and referrals/linkage to care); At least 75 percent of the award (after reserving amounts for administrative costs, planning/evaluation, and clinical quality management (CQM)) must be expended on core medical services costs (Please note: EIS is a subset of this 75 percent of the award) and; Not more than 10 percent of the total RWHAP Part C award funds can be expended on administrative costs. Applicants seeking a waiver to the core medical services requirement must submit a waiver request with this application as Attachment 15. For more details, see Program Requirements and Expectations.

Research shows that HIV care must increasingly address the management of individuals with multiple coexisting diseases (p.357) such as infectious and chronic diseases, mental health and behavioral disorders, food insecurity, substance use, etc. Since individual diseases do not exist in a vacuum, neither does the management, or burden of disease on community well-being. In response, syndemic theory and approaches are used to (1) increase the focus on co-occurring and interacting disease and social conditions that exacerbate health inequities, and (2) advance the evidence base for effective approaches to assess, monitor, and intervene with respect to these co-occurring and interacting factors for optimal outcomes in HIV treatment and care. The purpose of this project is to design, implement, and evaluate innovative comprehensive interventions part of an integrated treatment plan , using a syndemic approach to address co-occurring and interacting conditions of HIV, substance use disorder, intimate partner violence (IPV), and other social determinants of health that impact health outcomes of racial and ethnic minorities with HIV.

The NCCC, using a combination of various funding sources, provides services through telephone-based warmlines, a hotline, and internet-based educational consultation services in the following clinical areas: General HIV prevention, care, and treatment, including diagnosis, testing, and antiretroviral therapy. Pre-exposure prophylaxis (PrEP). Post-exposure prophylaxis (PEP). Treatment and management of Hepatitis B and C and HIV coinfections. Treatment and management of substance use disorders in people with or at risk for HIV. Perinatal HIV care and management. The NCCC provides expert consultation that demonstrates an understanding of the HIV epidemic in a variety of locations including: EHE jurisdictions. Rural areas. Areas disproportionately impacted by substance use disorders including opioid use disorder, and other co-morbidities. Communities where a large number of the people with HIV identify as a non-white racial or ethnic group. The goals for the NCCC align with those of the RWHAP Part F AETC Program, which are: Goal 1: Expand the number of health care team members providing HIV care and prevention services, including providers with different backgrounds or experiences. Goal 2: Expand the ability of health care team members to provide effective HIV care and prevention services. Goal 3: Improve health equity by integrating HIV care and prevention in primary care and other health care settings that provide services to underserved populations. Goal 4: Enhance the capacity of the AETC Program to train health care team members to serve people at risk for or with HIV.
