The program is designed to advance efforts that result in tangible and sustainable transformation of policies, systems, and environmental conditions. This Request for Proposals (RFP) is issued by the New Jersey Department of Human Services (DHS), Division of Aging Services (DoAS) and seeks bidders to provide the services described in this RFP beginning December 31, 2024 and ending on June 30, 2026. Contingent upon available appropriations, and with funding and oversight provided by DoAS and its contracted partners, grantees will undertake initiatives through one of two possible grant options: Age-Friendly Communities or Age-Friendly Projects. Each proposal should identify the primary bidder and any partners that will be funded by this grant, if awarded;  The bidder must be in good standing with DHS if it has an existing grant or contract in place. If applicable, the bidder must have all outstanding Plans of Correction (PoC) for deficiencies submitted to DHS for approval prior to submission;  The bidder must be fiscally viable based upon an assessment of the bidder’s audited financial statements. If a bidder is determined, in DHS’ sole discretion, to be insolvent or to present insolvency within the twelve (12) months after bid submission, DHS will deem the proposal ineligible for grant award.
